1. SMS Program Description

Identity Integrative Health operates an SMS messaging program to communicate directly with current patients, new patients, and individuals who have expressed interest in our services.

Messages sent through this program may include new patient welcome messages and onboarding information, appointment reminders and scheduling updates, care follow-up messages and progress check-ins, patient education content related to chiropractic, physical therapy, and fitness, health tips and lifestyle recommendations, Google review requests, referral invitations, information about our performance care and fitness programs, and practice news and updates.

This is an informational and care-support messaging program. It is not intended for emergency communications. For medical emergencies, please call 911.

2. How to Opt In

You may opt in to our SMS program by submitting your phone number and checking the SMS consent box on our new patient intake form, verbally consenting and having a team member record your opt-in, or texting a designated keyword to our SMS number (if applicable).

Consent to receive SMS messages is optional and is not required to receive care or services from us.

3. How to Opt Out

You may opt out of our SMS program at any time by replying STOP to any message you receive from us, or contacting us directly at (913) 387-4921 or hello@identityih.com to request removal.

Upon opting out, you will receive one final confirmation message and will be removed from our SMS list immediately. No further messages will be sent.

5. Message Frequency

Message frequency varies based on your stage of care and engagement with our practice. You may receive up to 4–6 messages per month during an active care plan, and fewer messages during maintenance or follow-up periods.

6. Message & Data Rates

Standard message and data rates may apply. Rates vary depending on your mobile carrier and service plan. Identity Chiropractic LLC is not responsible for any charges incurred by your carrier related to SMS messages.

7. Carrier Liability Disclaimer

Mobile carriers (including but not limited to AT&T, Verizon, T-Mobile, and others) are not liable for delayed or undelivered messages. Message delivery is subject to carrier network availability and conditions.

8. Age Restriction

Our SMS program is intended for individuals 18 years of age or older. By opting in, you confirm that you are at least 18 years old. We do not knowingly send SMS messages to individuals under the age of 18.

9. Privacy & Data Use

Your phone number and SMS consent information will be used solely to operate this messaging program. We do not sell, share, or transfer your SMS opt-in data or phone number to third parties for marketing purposes. 10. Not Medical Advice

The content on this website and in our SMS communications is provided for informational purposes only. It does not constitute medical advice, diagnosis, or treatment. Always seek the advice of a qualified healthcare provider with any questions you may have about a medical condition.

11. No Provider-Patient Relationship

Use of this website or participation in our SMS program does not create a provider-patient relationship. A provider-patient relationship is only established when you have been seen in person and a formal intake has been completed.

These Terms of Service are governed by the laws of the State of Kansas, without regard to its conflict of law provisions. Any disputes arising under these terms shall be resolved in the courts of Johnson County, Kansas.
