Your Right to a Good Faith Estimate

Under federal law, health care providers must give patients who don't have insurance, or who aren't using insurance to pay for their care, an estimate of expected charges before treatment. Because Identity Integrative Health is a cash-based practice, this applies to your care with us.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency item or service, including related costs like exams, treatment, and any recommended care plan.

When You'll Receive Your Estimate

You can request a Good Faith Estimate at any time, before or after scheduling a visit. If you schedule a service at least 3 business days in advance, you'll receive a written estimate within 1 business day of scheduling. If you ask for an estimate before scheduling, you'll receive one within 3 business days of your request.

Payment Methods

We accept HSA and FSA cards, all major credit and debit cards, and cash or check. If your insurance plan includes out-of-network benefits, we can provide a superbill so you can submit for reimbursement directly with your plan — reimbursement varies by plan and isn't guaranteed.

$90–$220, depending on the services provided

Comprehensive care plans (optional, paid in full)

Integrated — $972 · Integrated+ — $2,160 · Hybrid — $2,400

If Your Bill Is Higher Than Expected

If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill. Make sure to save a copy or photo of your Good Faith Estimate.
